According to the Centers for Medicare and Medicaid Services (CMS), the Quality Indicators, developed by the University of Wisconsin, represent common conditions and important aspects of care. These Quality Indicators reflect a measure of the prevalence or incidence of certain conditions based on a core set of screening, clinical and functional status elements determined by CMS.
The tables listed detail the overall results of the measurements and staffing reports from the past four quality surveys for each home. Some of these homes may be listed on our National Watch List.  Homes on the MemberoftheFamily.net National Watch List have had recent survey violations or have had substantiated complaints of actual harm or immediate jeopardy.
